Context

Describe the forces, constraints, affected stakeholders, and architecture characteristics that make this decision significant. State known assumptions and the problem, not a preferred implementation.

Decision

State the decision and its boundary unambiguously. Emphasize why it was chosen; include implementation detail only where it is itself a constraint.

Consequences

List beneficial and adverse consequences, trade-offs, operational effects, and follow-up work. Record meaningful alternatives that were rejected and why.

Compliance

Name the automated test, CI job, review check, or measurable condition that shows the decision still holds. If temporary variances exist, list their exact scope and removal condition.
